Advice for building a habit and in turn losing weight

Hi, I am a 20 F, 5’11, CW/SW: 340, (First) GW: 250, (Second) GW: 190. I am a homemaker. hi have high BP

First thing that I would like to say, is thank you to anyone who would give me advice. Also, thank you for reading. I feel like I can do this, but I want to know what worked for others.

What I have done so far: I talked to a nutritionist, and she went over some things that I can do. She recommended switching some of my daily foods. Such as 2% milk to skim, white bread to wheat, and white rice to brown rice. She recommended more fiber/protein. We calculated what would be a good amount of calories a day for me, and we discussed different portion sizes/food groups. We discussed exercise briefly also. I downloaded “Lose it!” and went on a 30 minute walk today. I have been tracking what I’ve been eating for about a week now, and I have not felt hungry since making the small changes she told me about. I have tried to stick to the calorie limit we set. I also have cut out or replaced my snacks with “healthier” snacks. I also try to measure out things accurately. I have also been on mental health medication for a while.

What I am wanting advice on: I am wondering what worked for others to keep going/keep the weight off. What kinds of daily things should I try to do in order to build a habit? What kinds of workouts would be good for someone at my weight? What are some good resources, or ways to keep disciplined? What are some switches I can make in my diet that would be healthier?

I am desperate for advice and anything would be appreciated. Thank you
