How do you permanently change your eating habits?

For me, having lean meats with vegetables before bed (or any other time) is a nightmare. I just love my tasty foods and carbs. I have never gotten used to consistently eating healthy, reducing my carb intake, and eating/sleeping light. I almost never eat breakfast, frequently eat street food, and enjoy alcoholic beverages with friends. I think I've gained about 25lbs since I slacked off on my endurance training (swim-bike-run). What's certain is that these food habits have been a part of my behavior for as long as I can remember. I just can't see myself coming home from a workday and having vegetables or fruit before bed just to do it all over again in the morning.

Has anyone here every successfully changed their daily dietary habits for the better? Any tips from experience would be highly appreciated.
